Privacy Policy and Notice of Privacy Notes

Trusted Care Partners is committed to protecting your privacy and ensuring your personal information is handled securely and responsibly. This Privacy Policy includes our Notice of Privacy Practices, as required by the Privacy Regulation created under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

NOTICE OF PRIVACY PRACTICES

As required by the Privacy Regulation created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), this section outlines how we may use and disclose your protected health information (PHI), as well as your rights regarding that information.

  • Request Restrictions: You may request limitations on how we use or disclose your PHI.
  • Confidential Communications: You may request that we communicate with you through alternative means or at specific locations.
  • Accounting of Disclosures: You may request a list of disclosures of your PHI made by us, excluding those for treatment, payment, or healthcare operations.

Our Responsibilities

We are required by law to:

  • Notify you promptly if a breach occurs that compromises the privacy or security of your PHI.

Our caregivers offer meaningful companionship by engaging in conversation, playing games, pursuing hobbies, and participating in enjoyable activities. This service helps your loved ones stay socially active and connected. 

General Privacy Practices (Non-PHI)

In addition to HIPAA-protected information, we may collect and use personal information such as your name, email, and phone number for the purposes of providing services, including SMS notifications.

Data Collection

We collect personal information, including:

  • Name, phone number, and email address.
  • Information entered through our online forms, including consent for SMS notifications.

Data Usage

We use this information to:

  • Send updates, respond to inquiries, and deliver services via email or text messages.
  • Provide SMS notifications for appointment reminders, service updates, or promotional offers.


Data Protection

We offer specialized care for individuals with dementia and Alzheimer’s disease. Our caregivers create structured routines, engage in memory-enhancing activities, and provide compassionate support to manage behavioral changes and reduce confusion. 

User Rights and Consent

  • Opt-In: Users are sent an opt-in text message once Trusted Care Partners are contacted and need for further communication has been established.
  • Opt-Out: Users can opt out at any time by replying “STOP” to SMS messages or contacting us directly.
  •  Message frequency may vary.

Opt-In for SMS Notifications

When contacting us via phone or email, users may choose to receive SMS notifications. Consent is obtained upon initial contact, and an opt-in message is sent.

Changes or Updates

We may update this Privacy Policy and Notice of Privacy Practices as necessary to remain compliant with applicable laws and best practices. Updates will be posted on this page with a new effective date.
