Trusted Care Partners is committed to protecting your privacy and ensuring your personal information is handled securely and responsibly. This Privacy Policy includes our Notice of Privacy Practices, as required by the Privacy Regulation created under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

As required by the Privacy Regulation created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), this section outlines how we may use and disclose your protected health information (PHI), as well as your rights regarding that information.

PHI includes any health information that identifies you or could reasonably be used to identify you, and that is transmitted or maintained by us in any form (electronic, paper, or oral).

We may use and disclose your PHI without your authorization in the following situations:

• Treatment: To provide and coordinate your healthcare or services.
• Payment: To bill and collect payment for services provided to you.
• Healthcare Operations: For administrative, quality improvement, and operational purposes.

We may also disclose your PHI as required by law, including:

• To comply with legal obligations, such as court orders or subpoenas.
• For reporting abuse, neglect, or domestic violence.

For uses or disclosures not described above, we will obtain your written authorization. You may revoke your authorization at any time in writing, except where we have already relied on it.

• Amend: You may request an amendment to your PHI if you believe it is incorrect or incomplete.
• Request Restrictions: You may request limitations on how we use or disclose your PHI.
• Confidential Communications: You may request that we communicate with you through alternative means or at specific locations.
• Accounting of Disclosures: You may request a list of disclosures of your PHI made by us, excluding those for treatment, payment, or healthcare operations.

We are required by law to:

• Maintain the privacy and security of your PHI.
• Notify you promptly if a breach occurs that compromises the privacy or security of your PHI.

This service helps your loved ones stay socially active and connected. 

In addition to HIPAA-protected information, we may collect and use personal information such as your name, email, and phone number for the purposes of providing services, including SMS notifications.

We collect personal information, including:

• Name, phone number, and email address.
• Information entered through our online forms, including consent for SMS notifications.

We use this information to:

• Send updates, respond to inquiries, and deliver services via email or text messages.
• Provide SMS notifications for appointment reminders, service updates, or promotional offers.

We offer specialized care for individuals with dementia and Alzheimer’s disease. Our caregivers create structured routines, engage in memory-enhancing activities, and provide compassionate support to manage behavioral changes and reduce confusion. 

• Opt-In: Users are sent an opt-in text message once Trusted Care Partners are contacted and need for further communication has been established.
• Opt-Out: Users can opt out at any time by replying “STOP” to SMS messages or contacting us directly.
•  Message frequency may vary.

When contacting us via phone or email, users may choose to receive SMS notifications. Consent is obtained upon initial contact, and an opt-in message is sent.
